What you'll accomplish

By the end of this guide, new patients will receive a link to complete their intake forms on their phone or computer before they arrive — so when they walk in, their info is already in Dentrix or Eaglesoft and you skip the 15–20 minutes of paper form entry entirely.

What you'll need

  • NexHealth subscription (your office manager will need to set this up — ask them to sign up at nexhealth.com)
  • Integration with your PMS (Dentrix, Eaglesoft, or Open Dental) — NexHealth sets this up during onboarding
  • Admin access to NexHealth portal
  • Time needed: 1 hour initial configuration; ongoing check-ins are 5 min/day
  • Cost: Ask your office manager — NexHealth pricing is per practice

How-To Guide: Set Up Digital Patient Intake with NexHealth

Step 1: Log into the NexHealth dashboard

  1. Go to your NexHealth dashboard (the URL is given during onboarding)
  2. Log in with your credentials
  3. Look for Intake Forms or Patient Experience in the left menu

What you should see: A dashboard with sections for Forms, Scheduling, Messaging, and Analytics.
