What you'll accomplish
By the end of this guide, new patients will receive a link to complete their intake forms on their phone or computer before they arrive — so when they walk in, their info is already in Dentrix or Eaglesoft and you skip the 15–20 minutes of paper form entry entirely.
What you'll need
- NexHealth subscription (your office manager will need to set this up — ask them to sign up at nexhealth.com)
- Integration with your PMS (Dentrix, Eaglesoft, or Open Dental) — NexHealth sets this up during onboarding
- Admin access to NexHealth portal
- Time needed: 1 hour initial configuration; ongoing check-ins are 5 min/day
- Cost: Ask your office manager — NexHealth pricing is per practice

Step 1: Log into the NexHealth dashboard
- Go to your NexHealth dashboard (the URL is given during onboarding)
- Log in with your credentials
- Look for Intake Forms or Patient Experience in the left menu
What you should see: A dashboard with sections for Forms, Scheduling, Messaging, and Analytics.
